What to Expect
When you arrive, you will be greeted by our medical receptionist who will kindly ask you to complete the patient forms packet.
You can save time in the office by completing the required forms marked before your appointment.
Patient Forms:
These are two (2) separate forms so be sure you fill out each one and print both of them and bring them with you to your appointment. These forms can be filled out online, then printed to bring with you to your appointment. If you’d prefer, you can print them, then complete them by hand.
What to Bring
Your Visit
If you are a new patient, please plan to arrive at least 15 minutes early to help ensure your records have the important information we need. We know things come up unexpectedly and we understand. Anew Vision Eye Specialists requires 24-hour advance notice to avoid any cancellation fees.
Dilation is necessary for a complete examination of the eye. Your pupils may require up to 30 minutes to dilate. Since your pupils may remain dilated for a full day after your visit, we recommend that you bring sunglasses to reduce your sensitivity to light. Your ability to read will be temporarily decreased. We also recommend you have someone drive you home.
